2023 Online Check In

All teams must complete the mandatory Online Check In. There is no onsite check in.

You will receive an email from SincSports to complete your check in, or you can start here.

Deadline: Thursday, September 7th at 8:00 pm

Packet: When you complete your online check in, you will print a packet that will include a copy of your SincCertified roster for each game.
You need to have your Official PRINTED player passes for all rostered players and guest players. You must have actual player cards or a pdf of the cards with you to be checked against the Sinc certified roster at each game.
Please have your medical releases with you at all times in case of an emergency.

•  2023/24 Player & Coach Passes 
   
• Guest Players (no mixed association guest players) 
  > from within club - Must provide Player Pass  
  > from another club - Must provide Guest Form from association.
• IYSA Guest Form
   
• IYSA Tournament Roster  
   >Save some time and populate the roster within Online Check In  Video. 
   
• Risk Management Form (More info found at IYSA)      
                
• CLUB IYSA Communicable Disease Release of Liability 
• Player/Coach IYSA Communicable Disease Release of Liability 
• IL Medical Release Form (dated after 3/1/2023) US Club Teams & Non-IYSA teams Only
• Form NT - US Club Teams & Non-USYS teams Only
   
• Travel Permit for Out of State Teams  (not required for teams registered with US Club)
