2.) Players cards (laminated with pictures)
3.) Medical Release Form (team state forms or the alternate from our website)
4.) Permission to Travel (if out of state team) Non US Club teams
5.) Guest Roster (if applicable)
6.) Guest Player Cards (if applicable)
